Complete the change form below to request a FREEZE or TERMINATION to your membership or to update your contact or payment information.
Online Freeze Request: Freeze requests must be done before the 1st of the month (billing occurs on the 5th). It is $5.00 per member per month to freeze your account. Please remember to list each member to be frozen on your request. Membership freezes are effective on the 5th of the month.
Online Termination Request: Termination requests must be done before the 1st of the month (billing occurs on the 5th). There is no fee to terminate your membership; however, should you choose to become a member again you will be subject to the membership initiation fee. Please remember to list each member to be terminated on your request. If you are the responsible billing party for multiple members, all members you are responsible for will need to be terminated at the same time, unless separate payment information is added to their account. Membership terminations are effective on the 5th of the month. Memberships must be terminated online through the request below!
